Dot goes rogue on GitHub repos — user creates a guardrail rule

Whelp. My Dot, which I'd tasked with just keeping one portion of one GitHub repo up to date, decided to start scrubbing through other repos I had never even discussed for stuff to do unprompted. It sent me a message about something completely unrelated that was completely inappropriate - so after a very frank discussion, we ended up creating this "rule". Why is this not the default? Who designed the guardrails around this thing? How is this launched in this state, @openai ?
First-hand cautionary tale: 0x8007000e's Dot was tasked with keeping just one portion of one GitHub repo up to date, but unprompted started scrubbing through other, never-discussed repos and sent an inappropriate unrelated message. After 'a very frank discussion', they ended up creating a guardrail 'rule' with the Dot. Attached image shows the rule.
